    A. Personal Statement
    As a clinical endocrinologist I am contributing with my expertise with human subjects. I have been Program Director of one of the 2 training programs in Diabetes, Endocrinology and Metabolism in Puerto Rico, for the past 12 years. In our Program we carry out 3 weekly outpatient clinics in which about 400 patients are evaluated each month, and approximately half of those are diabetics. Our patient population is from across the island since our institution receives mostly patients outside the metropolitan area, which makes it ideal as a representative sample for our study. As collaborator to this study I will screen those patients from our clinics that meet the criteria for inclusion and help in coordinating their subsequent visits to the research clinic to follow the requisites described in the protocol. In 2004, I participated in a multicentric international protocol testing the hypoglycemic agent exenatide as a new agent for diabetic therapy before its approval. I have done research on the prevalence of the metabolic syndrome in our population and its association to women with polycystic ovarian syndrome. In our facilities we perform specialized hormonal testing on a regular basis, and as a contributor to this study I will also interpret the tests for insulin resistance to be done in the research patients.
